Background: Adolescents with depression who engage in non-suicidal self harming behaviors are more likely to adopt negative coping strategies when faced with negative events. Therefore, these patients should be introduced to positive coping strategies. Evidences have showed that mindfulness-based interventions can positively impact the psychology of patients with mental disorders. This study was to explore the impact of a combination of mindfulness therapy and mentalization-based family therapy (MBFT) on suicidal ideation in adolescents with depressive disorder. Methods: Eighty adolescent patients with depression and suicidal ideation admitted to our hospital from September 2021 to February 2022 were selected as subjects. They were divided into a control group and a study group using the random number table method, with each group comprising 40 subjects. The control group received MBFT, whereas the study group received both mindfulness therapy and MBFT. The psychological status and suicidal ideations of the two groups were compared before and after the intervention. Results: The psychological health scores of both groups of patients were lower after the intervention, with the scores of the study group being lower than those of the control group (P < 0.05). The scores on the suicidal ideation scales for both groups were lower after intervention, and the study group scored lower than the control group (P < 0.05). The absolute values of the differences in psychological health scale scores and suicidal ideation scale scores before and after the intervention were higher in the study group than in the control group (P < 0.05). Conclusion: The combination of mindfulness therapy and MBFT can improve the psychological condition of adolescents with depression, reduce their suicidal ideations, and help them develop a healthy and positive outlook toward life, making this method worthy of clinical recommendation. Introduction: Immigrant youth face heightened risks of substance use due to the stress associated with immigration and acculturation. While parental intervention can have a preventative impact on substance use, parents need to be well-informed about substance use and effective interventions that can prevent substance use among immigrant youth. Such interventions ought to be culturally sensitive, family-based, and targeted at the specific substances that are prevalent in a given context. Identifying and curating interventions that can empower parents in addressing substance use can help mitigate the risks that immigrant youth may face. Methods: This scoping review aimed to identify the types, characteristics, and effectiveness of family-based substance use intervention programs. Based on Arksay and O'Malley's guidelines, interventions included in the review must have met the following criteria: (a) was a family-based intervention aiming to prevent substance use; (b) targeted immigrant teens aged 12 to 17 years old; (c) was published in English; (d) originated from Australia, Canada, New Zealand, or the United States. The pinch table was used to synthesize included articles, after which studies were compared and categorized, and cross-cutting categories were identified. Results: After screening 4551 searched literature, 13 studies that utilized family-based interventions were included in the review. All interventions were face-to-face programs, and most interventions involved parents and youth as participants. Eco-developmental theory and active learning strategies were used by multiple interventions. Given immigrant families were target stakeholders, both deep structure and surface structure cultural adaptations were utilized. Interventions increased parents' knowledge and skills regarding substance use prevention and delayed substance use initiation among youth. Conclusion: From the review, it was evident that parents are an essential element in any program aiming to prevent or reduce children's substance use. Besides information about substance use prevention, the curriculum also involves parenting and communication skills for parents to understand the protective effects of family. Effective family-based interventions for immigrant youth require attention to parenting and immigration stress, while also considering cultural adaptation. Although structural family theory proposes that family boundaries play a significant role in marital relationships, limited empirical evidence supports this theoretical model. The present study tested structural family theory by examining the complex associations between intergenerational boundaries and adult children's marital satisfaction, as well as the indirect effect of family adaptability and cohesion. A nonclinical convenience sample of individuals (333 participants, 168 females) married for at least one year was recruited. Participants provided self-report data on demographic variables, family boundaries, family cohesion and adaptability, and marital satisfaction. The internal intrusion score (intrusion by families of origin on the nuclear family) of intergenerational family boundaries was negatively associated with marital satisfaction, while the external intrusion score (intrusion by the nuclear family on families of origin) was positively associated with marital satisfaction. The effect of intergenerational family boundaries on marital satisfaction was exerted indirectly through family cohesion and adaptability. There was a significant association between intergenerational family boundaries and adult children's marital satisfaction. This study contributes to the conceptual framework of structural family theory, enabling family therapy practitioners to better understand the pathways through which intergenerational family boundaries impact adult children's marital satisfaction. In their systematic review on parent illness representations in their children with anorexia nervosa (AN), Marchetti and Sawrikar (International Journal of Eating Disorders, 2023) integrate past research on the parental experience of the illness, investigating whether the Common Sense Model of Self‐Regulation (CSM) is a useful framework for understanding parental responses to AN. Given that family‐based treatment (FBT) is a first‐line treatment for adolescents with eating disorders and is based upon integrating parents into the treatment process, it is especially relevant to explore how parent illness representations may impact FBT. In this commentary, we describe how FBT is designed to support parents and address many of the common unhelpful beliefs and emotions associated with parenting an adolescent with AN. We include specific examples of how parents can be supported within FBT as manualized. We also examine parental representations that may not be addressed by FBT, and how these may impact treatment outcomes in FBT, possibly via the impact of expressed emotion. Future research is needed to determine if modifications to FBT are required to address parental representations of the illness. Autism Spectrum Disorder (ASD) is widely regarded as the most severe childhood behavioral disorder. However, society's lack of awareness regarding neurodevelopmental disorders, like autism, has led to a limited understanding of their profound impact on children and their families. Challenges include unique sensory experiences, communication and social interactions, strain on familial relationships, emotional toll on parents, and educational hurdles, which can significantly impact the well-being of individuals and their families. This case study explores the integration of Structural and Experiential Family Therapy in a neurodivergent family coping with ASD and Attention-Deficit/Hyperactivity Disorder (ADHD). Structural interventions focus on clarifying roles and redistributing caregiving responsibilities, addressing issues of communication, and power dynamics. Experiential interventions target emotion-focused techniques, empathy building, and cultural sensitivity training. This integration may promote lasting change in family dynamics and a call to empower parents within neurodivergent families through tailored interventions, contributing to a sustainable therapeutic approach. There is increasing evidence that highlights the benefits of Family-oriented Therapeutic Conversations (FAM-TC) for the patient and the family; however, studies show variability regarding the content and the way these interventions are offered. This may hamper its further development in clinical practice. This review systematically maps the available literature on nurse-led FAM-TC and offers a solid synthesis of the characteristic, effectiveness, and feasibility of these interventions. A systematic search in PubMed, CINAHL, Cochrane, Web of Science, PsycINFO, Trip (Turning Research Into Practice), BASE (Bielefeld Academic Search Engine), OATD (Open Access Theses and Dissertations), and ProQuest databases identified 37 studies. The interventions varied in interventionist nurses' profile, the intervention content, or the duration of the sessions offered. Most of the interventions showed beneficial effects on perceived family support and family functioning. This review offers suggestions for future studies, such as the inclusion of specific theoretical frameworks for intervention design, targeting both the patient and the family and offered by nurses with family nursing competency. This article presents research that explored the ways that systemic and family therapists might approach the support of families living with dementia. A constructivist grounded theory methodology was used to interpret interviews with four systemic family therapists working in the United Kingdom. All participants had professional and/or lived experience of dementia. Transcript analysis and theoretical sampling led to the development of five categories, each related to different aspects of considering systemic therapy in a dementia context. These categories were further developed into a 'systemically informed dementia orienteering' conceptual framework, which is presented in this paper as a learning resource. This paper reports on the results of a study exploring sibling stories of parental mental distress and the societal discourses that may have contributed to the adult siblings' sense‐making. Four sibling pairs were purposively recruited and interviewed, using a narrative inquiry approach. Two main plots were constructed through analysis of the participants' narratives: 'The story of us' and 'We are who we are because of what happened'. Results highlighted that the identity construction of each sibling and the collective identities of the sibling pairs are influenced by the parental mental health context. The findings also suggest a change in the siblings' meaning‐making from childhood to adulthood. These findings are considered along with the implications for systemic practice and research. This conceptual paper explores the surprising impact exerted by third elements on the escalation and de‐escalation of conflicts in business families. To this end, we draw on core concepts of Batesonian cybernetic theory to perform a reconceptualisation of pertinent literature on changing conflict dynamics gathered in family business research and consultancy. Based on this reconceptualisation, we identify explanations and key mechanisms for the impact of third elements on conflict dynamics. One objective of this study was to obtain the psychological functioning profile of adolescents involved in youth-to-parent aggression (YPA) situations, as well as their family functioning, analyzing variables that had not received empirical attention, such as emotional intelligence, parental criticism, the parent-to-child bond, or knowledge of family history. Another objective was to explore which predictors related to adolescent characteristics, parenting and family functioning were most useful for discriminating between YPA and non-YPA groups. Participants were two equivalent groups (one from a family therapy center specializing in the treatment of YPA and a group from the general population), with a total of 133 adolescents and 256 parents. Regarding the psychological functioning of adolescents, the results indicated low emotion regulation and anxious-depressive symptomatology of adolescents involved in YPA. The parenting style was characterized by high overprotection and low care. However, family criticism (AUC =.828) was identified as the best predictor of YPA, with the capacity to discriminate between YPA and non-YPA groups. Families involved in YPA may have a rigid disengaged family functioning and affectionless-controlling parenting style. The central role of family criticism in YPA was unknown. The current study contributes to the literature by showing the need for individual psychological treatment and family therapy of all members of the family as a whole involved in YPA, not just the adolescent aggressors. This article examines the measurement equivalence of the Spanish Differentiation of Self Inventory for Adolescents (S-DSI-A) in adolescent populations across Spanish-speaking countries (N = 5,552), including Argentina, Chile, Ecuador, Mexico, Peru, and Spain. Within the Bowen family system theory, Differentiation of self is one of the most recognized constructs to systemic researchers and couple and family therapy practitioners, with implications in relationship adjustment or mental and physical health. The study uses multigroup confirmatory factor analysis (MGCFA) to test for configural, metric, and scalar invariance of the S-DSI-A across the six samples. The results indicate that the S-DSI-A demonstrates configural and metric invariance, but scalar invariance is not achieved. These findings have implications for cross-cultural research on differentiation of self and highlight the importance of considering measurement equivalence when comparing results across different populations. Specifically, our results suggest that the S-DSI-A reliably assesses the latent variables of Emotional Reactivity (ER) and Emotional Cutoff (EC) uniformly across Spanish-speaking nations, but the mean scores for the ER and EC lack cross-sample comparability. As a profession, social work is concerned with the person in the social context; yet, interventions typically involve formal service provision while the informal network is rarely a focal point. This well-established approach has implications across all fields of social work practice, but perhaps nowhere more so than in the alcohol and other drug treatment and child welfare sectors, as both support socially isolated individuals and families. This article discusses the Mirror Families™ pilot, an innovation in practice introduced to assist women exiting residential treatment create a supportive and enduring social network for themselves and their children. We write this paper as DClinPsy staff who have implemented Association of Family Therapy and Systemic Practice (AFT) Foundation and Intermediate level accreditation embedded within DClinPsy training. We describe the context and landscape in which this has happened, the positives we believe it brings and the challenges we have faced along the way. We offer suggestions for courses on this journey of approaches, methods and techniques that we have found helpful, and reflect on our next steps. Within the paper we include the voice of a trainee who is completing this training currently and consider what clinical psychologists in the NHS can do to support trainees and courses in achieving systemic secondary accreditation. This study examined the association between congruence in the two partners' perceptions and their relationship satisfaction. We considered congruence between metaperception and partner‐perception (meta‐partner) and between metaperceptions and self‐rated personality (meta‐self). The participants were 236 heterosexual couples. Each couple member reported couple satisfaction and personality traits (in terms of self‐perception, partner‐perception, and metaperception). The actor–partner interdependence model (APIM) revealed that individuals who believed to be viewed by their partner as more agreeable, conscientious, or emotionally stable than their partner saw them, were less likely to have a partner who was satisfied with the relationship. When metaperceptions of "Agreeableness" were higher than self‐perceptions, relationship satisfaction for both the actor and the partner was higher. This paper presents an analysis of how three interlocutors sequentially organize and accomplish mutual understanding in naturally occurring audiovisual recordings of therapy sessions. The analysis is in keeping with microanalysis of face-to-face dialog (MFD) and follows operational definitions of three-step micro-processes that interlocutors use when they calibrate new information; that is, how they agree that they have understood each other's words and actions well enough for current practical purposes. Pointing to some of the complexities that characterize triadic interactions, the analysis contributes with new documentations of 'suspended', 'nested', 'branched', 'multi-paced', and 'mixed interpretations' calibrations. The analysis also demonstrates how interlocutors may calibrate the 'tone' of an utterance before the topical content is mutually understood. The results and their implications may be relevant to practitioners of institutional talks at large, where the quality and outcome of, for instance, assessments and interventions largely rely on accomplishing mutual understanding. Young children's display of emotional and behavioral difficulties is relatively commonplace. However, if left unaddressed, these challenging behaviors have potential to develop into more serious problems. Furthermore, parents often report feeling unprepared to prevent and address their young children's emotional and behavioral difficulties. Fortunately, behavioral parent training strategies, such as Parent–Child Interaction Therapy, have been found to be effective for helping parents learn to effectively improve young children's behavior. Although effective, there are barriers to accessing behavioral parent training, such as cost, lack of transportation, and living in rural areas with limited mental and behavioral health care. Therefore, the purpose of this pilot study was to evaluate the effects of internet‐delivered PCIT (iPCIT) on parents' use of praise, reflections, imitations, descriptions, and enjoyment/enthusiasm (PRIDE skills) with children referred for disruptive behavior. Participants in this study included three parents and their 2‐year‐old children. We used a concurrent multiple probe across participants' design to test the effects of iPCIT on parents' use of PRIDE skills and maintenance of treatment effects. Results indicated that all three parents increased their use of PRIDE skills and maintained them following termination of treatment. Results, implications, and future directions are discussed. The writer shares the course of treatment with a gender nonbinary teenager and their parents that utilized techniques from drama therapy, relational self psychology, sculpting, and family therapy. Many pioneering family therapists exuded exemplary personal characteristics that at many instances were incorporated into their clinical work and theoretical approaches to psychotherapy. While such therapist effects have been traditionally less researched compared with the vast literature on the efficacy of techniques, they are of increased empirical attention, especially given the common factors of psychotherapy change and related process research. To explicate these therapist effects, empirical studies within the fields of marriage and family therapy specifically and psychotherapy more broadly are reviewed concerning those personal characteristics that are linked to or are postulated to be associated with therapeutic success. Following a brief synthesis of this literature, two family therapy pioneers, Carl Whitaker and Salvador Minuchin, are reviewed, concerning their personality attributes and characteristics that were highlights of their approaches and their personal and professional lives. Secondary traumatic stress (STS), or experiencing trauma through exposure to clients' traumatic stories, occurs across helping professions. No studies have focused solely on STS in Marriage and Family Therapists (MFTs). In a sample of MFTs (N = 201), this study hypothesized that exposure to trauma through therapeutic work would be associated with STS and MFTs' intentions to leave their job. Findings indicated trauma exposure was positively associated with STS (β =.33, p <.001) and intention to leave (β =.18, p <.001). STS partially mediated the association between MFTs' exposure to trauma in their work and intention to leave (β =.06, p <.05). Compassion satisfaction (β = −.49, p <.001) and organizational commitment to resilience building (β = −.26, p <.001) were negatively associated with intention to leave. An exploration of peer-reviewed articles published in the last decade in 11 journals from the American Counseling Association (ACA) was conducted to identify the state of research on teaching family and systems theory in the field of counselor education. This study presents a review of the relevant literature using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework. While much has been published over the past decade specific to family and systems theory, few recommendations exist for counselor educators attempting to select the most recent and relevant publications to include in their counselor education curricula. Each year, millions of marriages end up in divorce which can cause teenagers to experience multiple physiological and psychological issues. Cognitive Behavioral Family Therapy (CBFT) has become increasingly popular among family therapists in recent decades, who utilize it either as a stand-alone technique within a system perspective or integrated into other approaches with couples and family therapy. There is a gap in the literature regarding the utilization of CBFT to a divorced family of Hispanic/Latinx descent with an adolescent who developed an eating disorder. The overall aim of this article is to provide insight for marriage and family therapists (MFTs), other mental health professionals, professors/teachers, researchers, and virtually everyone. The researchers hope to provide the ability to conceptualize eating disorders in a way that promotes empathy, compassion, and awareness. Over the past three decades, "lived experience" designated roles in mental health services in Australia have increased. However, the lived experience of staff in roles not designated in this way is often hidden or unacknowledged. This qualitative study included 33 participants employed in a range of roles at two Victorian mental health services, including 10 social workers. Interviews explored staff perspectives regarding the sharing of lived experience by staff with colleagues and supervisors. Concept analysis identified conflicting ways of thinking about lived experience, which influenced when and how staff shared and how they responded to sharing by other staff. These findings suggest the need to explicitly address the purpose and value of sharing lived experience in the mental health workplace, to better support staff and service users. Recommendations for social workers and their colleagues, working at all levels of the mental health system, are provided. IMPLICATIONS The perspectives and experiences of mental health service staff suggest room for improvement in the valuing and inclusion of staff with lived experience, which may have benefits for service-user experiences of support. Promoting the value of sharing lived experience to reflective-practice and service reform is consistent with core social work values and, thus, the responsibility of all social workers, supervisors, and educators. Objective: The Systemic Therapy Inventory of Change (STIC) is a systemic measurement feedback system that provides therapists with feedback regarding the multidimensional clinical change in individual, couple, and family therapy. The STIC Intersession scales include Individual Problems and Strengths (IPS), Relationship with Partner (RWP), Family/Household (FH), and Child Problems and Strengths (CPS). They are administered to clients before each therapy session. The purpose of the current study is to investigate the STIC Intersession scales' sensitivity to change, the ability to detect reliable and valid changes that occur after an intervention. Method: Participants (N = 583) who voluntarily received individual, couple, or family therapy services in a randomized clinical trial attended the study. Results: By comparing the changes in pre-therapy and post-therapy scores of the STIC Intersession scales with those of the corresponding reference measures, the external sensitivity to change of the STIC Intersession scales was supported. The IPS Intersession scale showed greater change than the Beck Anxiety Inventory. However, no evidence supported the discriminant validity of CPS's change scores. Conclusion: Thus, the STIC Intersession IPS, RWP, and FH can be validly used to assess multi-systemic changes in both research and clinical work. [ABSTRACT FROM AUTHOR]

The present study explored the acceptability, feasibility, fidelity, and outcomes of a young adult adaptation of multidimensional family therapy (MDFT), an evidence-based family treatment originally developed for adolescents. Participants included 22 individuals between the ages of 19 to 25 who were enrolled in a criminal drug court program. MDFT was found to be feasible and was delivered with strong fidelity to young adults and their families. Participants reported high satisfaction with MDFT, and 95% completed treatment. Analyses revealed statistically significant decreases in substance use on all indicators from baseline to the 6-month follow-up. Significant improvements were also noted in vocational functioning, including a 73% increase in full-time employment from baseline to 6-month follow-up. Criminal justice outcomes included a significant decrease in legal risk, and 86% of study participants had no rearrests from baseline through the 18-month follow-up period. Background: Glycated hemoglobin (HbA1c) control is a crucial goal in the management of type 2 diabetes mellitus (T2DM), requiring lifelong commitment and family support. This study aimed to assess the effectiveness of family-based diabetes management intervention on HbA1c among adults with T2DM. Methods: From inception up to 2022, a comprehensive literature search was conducted across PubMed, ProQuest, Scopus, CORE, and the Cochrane Library. The quality of studies was evaluated using the Joanna Briggs Institute (JBI) Critical Appraisal tools. Effect sizes were calculated using standard deviations (SD), while the degree of heterogeneity was evaluated using the Higgins I2 test. Subgroup analyses were performed to explore factors contributing to sources of heterogeneity among trials. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) were followed, and the protocol was registered with PROSPERO CRD42022384034. Results: A total of 18 randomized controlled trials (RCTs) involving 2815 participants indicated that family-based diabetes management intervention had a statistically significant impact on improving HbA1c (Mean Difference [MD] = −.47; 95% Confidence Interval [CI]: −.64 to −.30, p <.001) with a moderate level of heterogeneity (I2 = 59%). Subgroup analysis indicated that family-based diabetes management intervention among adults with T2DM in developing regions was more effective in improving HbA1c levels compared to developed countries. Conclusion: Family-based diabetes management interventions improved HbA1c. Theological/spiritual reflection in psychotherapeutic practice has increased in recent years. Approaches for reflection and integration vary depending on the practitioner's spiritual and theoretical beliefs. The integrative approach utilized in this paper is derived from a phenomenological perspective of the author, who was schooled in pastoral theology and later family therapy. Considering the pastoral theologian Seward Hiltner's perspectival approach, this integrative approach creates a conversational method, integrating the client's concerns with specific narrative therapy interventions or practices and the theological/spiritual concepts of immanence–transcendence. Finally, this case study's methodology offers constructive questions that clinical practitioners can apply to specific psychotherapy approaches as well as theological concepts. Family systems therapy originated in the 1960s, 1970s, and 1980s through the work of innovative thinkers and clinicians. However, despite the creative contributions of the mentioned colleagues and of later innovations in family therapy theory and practice, it seems as though the dominant culture of establishment psychiatry in the United States (and in most Western countries) to this day has not seriously incorporated relationships, social context, or community connectedness into the treatment of individuals with psychiatric diagnoses. For the "psychiatric" patients diagnosed according to the DSM‐5, the dominant underlying epistemological perspective is the medical "scientific" paradigm. Within this approach there is a dearth of reflections about the truthfulness of so‐called "empirical facts" and a lack of skepticism about the techniques of "measurement" of the psychiatric illness. The alternative, relationship‐oriented, context‐sensitive, and community‐connected thinking paradigm is highlighted here in contrast to the "psychiatric" foundation. This paradigm consists of (a) the awareness that all human Subjects (including "psychiatric" clients) are constituted as such by their relational connection to others; (b) the awareness of our sensitivity to and embeddedness in a socio‐economic, cultural, and racial context; and (c) the awareness of our involvement in and connectedness with many kinds of communities. Couple therapists have the unique and challenging opportunity of helping people find deeper connection in their intimate relationships. These clinicians apply therapeutic models and interventions designed to help couples. However, many of these models are derived from theoretical, scientific, and sociocultural traditions that conceptualize human phenomena as individualistic and reductionistic, and the language in these theories may not match clients' experiences, which include deeply relational phenomena such as love and loss. We review how Western scientific and sociocultural traditions have shaped conceptualization of problems and clinical intervention in couple therapy. In contrast, we provide an alternative, strongly relational framework for couple therapy that draws upon philosophical perspectives asserting that relational, rather than individual, experience is fundamental in human interaction. These ideas provide relational language which can influence how couple therapy is viewed and enacted. Specifically, we discuss how a strongly relational approach changes how therapists understand and intervene with conflict, abuse, love, and deception. This relational framework can help couples in their own goals to become more unified. Implications for the therapeutic relationship, interventions, and scholarship are provided. This article describes child‐oriented family therapy (COF), a family therapy approach to early psychological and behavioral problems that is widely applied in northern Europe and has been adopted by countries with different cultures, such as Germany and China. This article explains the theoretical background, setting, characteristics, development and effective factors of this method and explores relevant research. In addition, the application of this method in China and reflections on its applicability to families in different cultures are presented. A clinical case is provided to illustrate the core therapeutic principles of COF. Since the application of COF interventions to families with children in Asian cultures remains understudied, the widespread availability of COF and adaptation to local conditions could facilitate training and research achievements in this field. Writings on Open Dialogue approaches to working with families experiencing mental distress emphasize the importance of the therapist repeating the family's words back to them verbatim. Repeats are theorized to display the therapist's listening and to encourage elaboration without imposing the therapist's own interpretations or conclusions on the family. These recommendations have not been subjected to rigorous interactional investigation. In this study, we used conversation analysis to analyze 160 examples of therapists repeating the prior talk of the family from 14 h of video‐recorded Open Dialogue sessions. We found that these repeats had similar functions to those previously described in conversation analysis research such as initiating repair, marking receipt, and requesting elaboration as well as examples of therapist repeats occurring in overlap with the talk of the client. Open Dialogue writings thus present an oversimplified account of how repeats are utilized in clinical Open Dialogue sessions. Consistent with dialogical writings, repeats can elicit elaboration with limited input from the therapist, however repeats reflect selectivity and function to direct and guide the conversation to some extent. So, while therapist authority can be mitigated by repeating clients' words, when a therapist chooses to employ a repeat actively shapes the direction of the interaction. [ABSTRACT FROM AUTHOR]

The Ackerman Institute for the Family, established in 1960 by Dr Nathan Ackerman, stands as one of the oldest and most respected family therapy institutes in the United States. Ackerman pioneered the integration of systemic insights into group settings, emphasised the crucial role of family in therapy, and advocated for the advancement and acceptance of family therapy. 'The Ackerman' played a pivotal role in launching Family Process, the first journal dedicated to academic activities in family therapy. Diversity and inclusion have been central tenets of Ackerman philosophy, evident in its programs, training courses, and staff composition. This commitment has produced a veritable cadre of family therapy leaders who have contributed significantly to both the Ackerman Institute and the profession, influencing policy decisions and clinical practices. One of the most esteemed and respected thought leaders and innovators of our field, who played a vital role in the legacy of several institutions, including the Ackerman, is Dr. Evan Imber‐Black. She served as a long‐time faculty member and director of the Ackerman Center for Families and Health. Her expertise encompasses the exploration of family rituals and family secrets, with a focus on the importance of rituals in providing meaning, identity, and connection across diverse cultures and family life cycles. As editor of Family Process for 8 years, she highlights the importance of examining scholarly work in the context of cultures as a requirement – we do not stand outside the culture – we are active participants. This paper is based on a conversation with Dr Evan Imber‐Black, held in June 2023, delving into her personal and professional experiences, her connection to the Ackerman Institute, and her influential work on family rituals and secrets. Umberta Telfener is a highly respected figure known for her diverse contributions to various facets of family and systemic therapy. Her leadership style has earned her a reputation as somewhat of a 'cultural anthropologist,' reflecting her aptitude for creating connections, establishing relationships, and developing partnerships. Her unique 'Umberta style' is known for boundless energy, active leadership, and fierce commitment to challenging the status quo. Despite being in office for just a year at the European Family Therapy Association (EFTA), Umberta has conceived and developed numerous projects, establishing networks with practitioners, and systemic thinkers across the world. Rooted in classical philosophical training, her pursuit aligns with the systematic study of fundamental questions about existence, reason, knowledge, value, mind, and language. Umberta's role reminds one of a 'Chasqui,' a Quechua term for a 'messenger,' which aptly describes her agile networking skills. She actively communicates and shares vital messages, supporting the international community of systemic practitioners. This paper is based on an in‐person conversation held in July 2023 at Umberta's country home in Tuscany. Our discussion delves into her extensive professional experience, from training at the Philadelphia Child Guidance Clinic, being mentored by Boscolo and Cecchin in Milan, to her leadership role as the current EFTA president. Several Euro‐American approaches to couple and family therapy have been instrumental in promoting successful practice of couple and family therapy in continental Africa. This article, however, describes one instance in which an African couple's distress of one‐dimensional fertility could not be resolved by drawing solely from the Euro‐American family therapy tradition. One‐dimensional fertility is defined as a crisis that arises in conditions of sonlessness or daughterlessness in a marriage, that is, in situations where there are only male or female children born to the marriage. The use of medical intervention through the prescription of drugs often worsens rather than enhances the psychological well‐being of couples who suffer from the distress of one‐dimensional fertility. Therefore, it is considered beneficial to encourage the development and recognition of psychological literature that suggests what to be done to control such a distress without recourse to the use of medication or the complicated and sensitive procedure of in vitro fertilisation. The article suggests that the leading intervention package for attending to the challenge of couples with the distress of one‐dimensional fertility entails the exercise of inducting them into the principles and practice of the fertility awareness‐based method of family planning known as the billings ovulation method that is in harmony with the African cultural perspective. This participatory research project highlighted the views and experiences of young people who are the brothers and sisters of children and young people who have experienced child sexual abuse. By working with young people who had experienced sexual abuse and their siblings, this practitioner research sought to give these young people a voice. The research involved a group process with young people who have experienced sexual abuse and have siblings, and one‐to‐one online consultations with the brothers and sisters of young people who have experienced abuse. All the young people spoke of their sibling and family relationships as unique and requiring unique responses to these relationships by services and health‐care professionals. Siblings' needs in the aftermath of a child sexual abuse discovery are often overlooked. This research describes how just as children who have experienced sexual abuse may feel anger, fear, guilt, shame, anxiety, and confusion, so do their siblings. Brothers and sisters of young people who have experienced sexual abuse describe significant relational trauma with intense emotional experiences of isolation, confusion, sadness, and anger. There is a real need to support young people and families to find words and develop capacities to talk about child sexual abuse ‐‐ a need to find supportive ways to speak about what has occurred and understand its impact on all. Background: Experiencing the illness and death of a child is a traumatic experience for the parents and the child's siblings. However, knowledge regarding effective grief interventions targeting the whole family is limited, including how to integrate age-appropriate support for siblings. Aim: We aimed to synthesize the empirical literature regarding grief interventions that target the whole family before and/or after the death of a child. Design: A scoping review following the Joanna Briggs Institute and Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines. Data sources: We searched PubMed, PsycINFO, Embase, CINAHL, and Scopus covering January 1998–May 2022. We included studies describing any type of structured intervention targeting the whole family (i.e. parents and siblings) before or/and after the death of a child (below 18 years), with pre-post assessments of grief-related symptoms in the family as an outcome. Results: After removal of duplicates, we screened the titles and abstracts of 4078 publications and identified 30 publications for full-text screening. None of the studies met the inclusion criteria. Most of the studies were excluded because they either did not target the whole family or did not target families who had lost a child below 18 years. Bereavement camps were a popular form of family intervention, but none were evaluated in a pre-post design. No grief interventions offering support pre-death were found. Conclusions: There is great need for research to improve bereavement outcomes for the entire family and to potentially integrate this in pediatric palliative care. [ABSTRACT FROM AUTHOR]

Objective: To examine the feasibility and acceptability of augmenting family‐based treatment (FBT) for adolescents with anorexia nervosa (AN) or atypical anorexia nervosa (AAN) with a parent emotion coaching intervention (EC) focused on reducing parent expressed emotion. Method: In this pilot effectiveness trial, families of adolescents with AN/AAN exhibiting high expressed emotion received standard FBT with either (1) EC group or (2) support group (an attention control condition focused on psychoeducation). Results: Forty‐one adolescents with AN or AAN were recruited (88% female, Mage = 14.9 ± 1.6 years, 95% White: Non‐Hispanic, 1% White: Hispanic, 1% Bi‐racial: Asian). Most study adolescents were diagnosed with AN (59%) while 41% were diagnosed with AAN. Participating parents were predominantly mothers (95%). Recruitment and retention rates were moderately high (76% and 71%, respectively). High acceptability and feasibility ratings were obtained from parents and interventionists with 100% reporting the EC intervention was "beneficial"–"very beneficial." The FBT + EC group demonstrated higher parental warmth scores at post‐treatment compared to the control group (standardized effect size difference, d = 1.58), which was maintained at 3‐month follow‐up. Finally, at post‐treatment, the FBT + EC group demonstrated higher rates of full remission from AN/AAN (40%) compared to FBT + support (27%), and were nine times more likely to be weight restored by 3‐month follow‐up. Discussion: Augmenting FBT with emotion coaching for parents with high expressed emotion is acceptable, feasible, and demonstrates preliminary effectiveness. Public Significance: Family based treatment for AN/AAN is the recommended treatment for youth but families with high criticism/low warmth are less likely to respond to this treatment. Objective: Family‐Based Treatment (FBT) is the leading manualized treatment for adolescent eating disorders; however, there is limited research on the adaptation of FBT for diverse families (i.e., families belonging to identity groups subject to systemic barriers and prejudices). The purpose of this qualitative study was to address: (1) adaptations made to the FBT model (if any) by clinicians working with diverse youth and families; (2) the barriers/facilitators of maintaining adherence (fidelity) to the model for these families; and, (3) the barriers/facilitators to access and engagement in FBT for diverse families. Method: Forty‐one FBT clinicians were recruited globally using purposive and snowball sampling, and listservs from eating disorder networks. Clinicians participated in individual interviews or focus groups, discussing their experiences delivering and adapting FBT for diverse families. Qualitative data was transcribed verbatim and analyzed using directed content analysis. Results: Some participants reported making adaptations to every phase of the FBT model, while others did not, when working with diverse families. In Phase 1, participants cited adapting the family meal, length/number of sessions provided, and addressed systemic barriers. In Phase 2, participants adapted the length of the phase and rate/level of independence given back to the adolescent. In Phase 3, participants increased or decreased the number of sessions, or eliminated this phase to address barriers to engagement in FBT. Discussion: This is the first study to qualitatively examine clinicians' experiences of implementing FBT with diverse families. Results may inform future FBT planning, clinician training, clinical decision‐making tools, and opportunities for modifications to the foundational model. Public Significance: This qualitative study examined clinicians' perceptions and experiences implementing FBT with diverse families, specifically what adaptations (if any) were made to the foundational model, and the barriers and facilitators to adhering to and engaging in the model. Despite common use of the Therapy Attitude Inventory (TAI) to measure satisfaction with parenting interventions, psychometric support has only been demonstrated in predominantly White caregivers of typically developing children following clinic-based treatments. Additionally, the reliability and validity of the Spanish version of the TAI have not been assessed. The present study examined the reliability and validity of the English and Spanish versions of the TAI among mostly Hispanic/Latinx caregivers of children with developmental delay (DD; n = 65) following Internet-delivered Parent-Child Interaction Therapy (iPCIT). Internal consistency was excellent for both versions. Convergent validity was demonstrated by a strong correlation with the Client Satisfaction Questionnaire-8. Satisfaction was predicted by improvements in child behavior and parenting skills, as well as reductions in caregivers' negative feelings about parenting, following treatment. These findings support use of the TAI with Spanish-speaking families, families from minoritized backgrounds and with children with DD, and following telehealth interventions. [ABSTRACT FROM AUTHOR]

This study aimed to identify the role of cooperation between medical personnel and the parents of adolescent patients with anorexia nervosa in stationary treatment. The medical documentation of 36 patients under 14 years old was analyzed using Consensual Qualitative Research methodology. Qualitative analysis allowed two parent groups to be distinguished: those cooperating with medical personnel and those with difficulties in collaboration. The BMI of children with cooperative parents was significantly higher at the end of treatment, with no difference at the beginning. Hospitalization was a difficult experience for parents and cooperation with personnel was burdened by mistrust, concerns, and doubts. [ABSTRACT FROM AUTHOR]

This study examined the effect of pre-treatment levels of parental expressed emotion (EE) on early treatment response for adolescent anorexia nervosa (AN). Data were collected from 121 adolescents, ages 12–18, who met DSM-IV criteria for AN excluding the amenorrhea criterion, and their parents. Participants were randomized to family-based treatment (FBT) or adolescent-focused therapy (AFT). To examine the effects of different thresholds of EE, we used two different levels of EE in analyses. Results demonstrated that adolescents who had at least one parent with elevated EE indicated by a lower threshold (i.e. even mild levels) at baseline were less likely to achieve an early treatment response, suggesting that EE might interfere with treatment success from the start of treatment. When high EE was defined by a higher threshold, these effects were no longer significant, regardless of treatment type (FBT or AFT). These findings suggest that adolescents with AN may be more sensitive to EE than other mental illnesses, such that lower thresholds of EE impact the speed with which they are able to reduce symptoms and gain weight in treatment. It may be necessary to target parental EE prior to or early in treatment or pivot to parent-focused treatment to change the trajectory of treatment response. Past research suggests behavioral treatments are effective for challenging behavior in children and young adults with neurodevelopmental disorders, such as autism spectrum disorder (ASD). However, access to these services can be limited and require substantial resources. To address this issue, the current study provides a programmatic description of an individualized parent-mediated service model targeting moderate challenging behavior. In the program, therapists coached parents to implement functional analyses and individualized function-based treatment packages. Forty-one families of children and young adults with neurodevelopmental disorders participated. Most clients (75.6%) met all admission treatment goals and parents reported significant decreases in frequency and severity of challenging behaviors at discharge. Parents also reported less stress at discharge. Outcomes of the study suggest this parent-mediated treatment model is a viable option to treating moderate challenging behavior in children and young adults with intellectual and developmental disorders. Preparing the future mental health workforce requires clinicians to be able to adapt their clinical work to different mental and medical healthcare settings to meet the growing need for mental health services. Medical Family Therapists (MedFTs) are specifically trained to provide care for patients from a biopsychosocial-spiritual perspective in diverse clinical settings. Having a theoretical foundation is a necessity for MedFTs to be able to adapt their clinical work to different contexts. Solution-focused therapy provides such a foundation. The need to expand knowledge of family-based intervention for people with ASD has been observed. However, there is a lack of study in Chinese context such as Hong Kong exploring the use of multifamily therapy (MFT) for adults with ASD. The primary aim of the present study is to assess the feasibility of a process-oriented MFT in promoting family relationships for young adults with high functioning-ASD (HF-ASD) and their parents. We also sought to explore the therapeutic process factors of MFT that were deemed helpful by the family participants in our study. Two types of MFT—one online, the other face-to-face—were conducted from March to August 2021 for families with adults with HF-ASD in Hong Kong. In total, 13 families participated in our multi-method study. Among the results, cross-family therapeutic alliance and treatment engagement were positively associated with enhanced family relationships among participants. Significant differences emerged in changes in the cross-family therapeutic alliance and family relationships between the online and face-to-face MFTs. Two themes regarding the helpful aspects of MFT emerged: first, having a cross-family interactional context and, second, the authentic engagement of people with HF-ASD. Overall, our results imply that MFT, in either online or face-to-face mode, could be a feasible family group psychotherapy for adults with HF-ASD and their parents. [ABSTRACT FROM AUTHOR]

As the bilingual Spanish-speaking population in the United States (U.S.) steadily rises (U.S. Census Bureau in Language spoken at home, https://data.census.gov/cedsci/profile?q=United%20States&g=0100000US, 2020), the need for systemic therapists who are competent in working with Spanish-speaking clients also increases. While it is unrealistic to require all mental health practitioners to be bilingual English and Spanish speakers, systemic therapists can improve their understanding of the impact that clients' native language can have on the therapeutic process. In this paper, we synthesize concepts from narrative therapy and linguistic relativity to provide non-Spanish-speaking clinicians with a unique perspective of case conceptualizations and therapeutic interventions for clients who natively speak Spanish. A clinical vignette is presented to illustrate the practical application of linguistic relativity informed systemic therapy. The purpose of this study was to document factors associated with a couple's decision to manage finances entirely jointly (i.e., pooled), somewhat jointly, or separately. Based on survey data from 636 married or cohabitating respondents, test results showed that married and less well-educated individuals are more likely to pool finances with their partners. Additionally, it was determined that the odds of partners pooling finances increase as the size of a household increases and when the household exhibits a positive net worth. In this study, households with two income earners were approximately 50% less likely to pool their finances compared to households with one income earner. It was further determined that those who reported agreeing on issues related to spending were more than twice as likely to pool their finances as compared to those who did not agree with their partner on issues related to spending. The findings from this study advance the marriage and family therapy literature by showing that financial integration style and financial decision-making responsibilities are separate constructs and should not be used as proxies for one another. Purpose: It is a half‐century since the coalescence of social psychiatry and systemic family therapy approaches started to inform condition‐specific therapeutic work with families to reduce relapse and hospital readmission for people with schizophrenia. Today, family interventions are a cornerstone of international guidelines for the treatment of psychosis, and of workforce development initiatives. Effect sizes for clinical and economic outcomes are large, and the evidence base is robust and reliable, not only for outcomes but also for the underpinning theoretical models, which are coherent and consistent. Few, if any, psychological therapies, have so powerful a framework to drive widespread implementation. Nevertheless, delivery in clinical services is variable, often lagging behind that of individual cognitive behavioural therapy, notwithstanding its considerably weaker implementation framework. Our aim in this article is to formulate this translation failure and offer potential solutions. Method: We summarise the model/intervention and supporting evidence, then consider why delivery remains problematic. Results: We highlight the inter‐linked issues of conceptual confusion between and conflation of, different approaches to working with families; of addressing diagnostic uncertainty, complex comorbidity and adapting interventions for specific populations; and of translation from gold‐standard research trial practice, through educational curricula and training programmes, to routine delivery in frontline services. Conclusion: We present our view of clinical, research and workforce development priorities to address these issues and continue the collective effort, moving into the next half‐century, to work more effectively with people with psychosis and their families, to further improve outcomes. Clinical care following a severe brain injury often requires a host of physical and psychological rehabilitation interventions. However, a brain injury affects the entire family. Therefore, a family-level approach to neurorehabilitation services should be favoured over an individual one. Social workers are often well positioned to assess and address family-level processes. By exploring key family-level theories, models, and approaches to brain injury intervention, this article provides clinical social workers with practical guidance. Illustrated through a descriptive case example, it serves as a user-friendly resource for application in real-world settings. The skills and tools discussed will likely be familiar even to those without practice experience in neurorehabilitation. The value of emphasizing clinical process skills related to common factors in therapy rather than content expertise related to brain injuries is discussed. The present study aims to investigate whether each coping style used by Vietnamese people living with infertility diagnosis is associated with specific types of infertility-related stress (IRS). In this cross-sectional design study, 997 patients with primary infertility diagnosis from three hospitals and two clinics in three regions of Vietnam completed questionnaire that consisted of Fertility Problem Inventory, the Copenhagen Multi-Centre Psychosocial Infertility and other questions. Four different linear regression analyses were performed on four coping styles. The five types of IRS and covariates were included in these models. The results show that participants who experience all five types of IRS reported the dominant use of active-avoidance coping, while having four types of IRS, except for social concern, was associated with higher use of meaning-based coping. Utilising active-confronting coping was reported to be the outcome of experiencing increasing need for parenthood and decreasing rejection of child-free lifestyle. Choice of passive-avoidance coping was more common among those with increasing social concern and need for parenthood. Age and educational level impacted infertile people's choice of avoidance coping strategies. The results provide evidence to understand the direct impact of each type of IRS on infertile people's choice of coping styles to better support them during their individual and family therapy. Immunodeficiency diseases associated with the absence or dysfunction of any of the components that make up the immune system. These diseases may create difficulties for family functionality. The study aims to examine family functionality in the context of McMaster Family Therapy in families with immunodeficiency patients. The study include 153 family members from Immunodeficiency Patients Solidarity Platform in Turkey. The sociodemographic questionnaire and McMaster Family Assessment Device were used to collect data with respect to problem solving, communication, roles, affective responsiveness and involvement, behaviour control, and general functioning. The majority of the participants are married, females, university graduates and low income. Most of them have nuclear families and a diagnosis of primary immunodeficiency in children. Areas of non-healthy family functionality include affective involvement and behaviour control. Also, problem-solving, roles, and communication create risks in terms of dysfunction.
